What is involved on Impairment Rating Evaluation and Independent Medical Evaluation?
Examinee will complete a comprehensive questionnaires and history forms which will be review with Dr. Nieves. Following the history portion of the evaluation Dr. Nieves will conduct a Physical Examination. Dr. Nieves will also review all medical records and diagnostic films provided by the referring source. Examinee is responsible to bring diagnostic films and any additional records that he or she might want Dr. Nieves to review and include in the report. Those additional records and films must be delivered before the evaluation or no later than the day of the evaluation. There is no doctor / patient relationship on Independent Medical Evaluations and Impairment Ratings for which Dr. Nieves will not be answering any questions to the examinee or providing the examinee with any treatment or recommendations that might be construed as establishing a doctor / patient relationship.
Who gets the report?
Once the report is completed a copy is send to the referring source and the examinee will get a copy from the referring source.
What is involved in a Work Related Injury or Motor Vehicle Accident evaluation for diagnosis and treatment with Dr. Nieves?
Comprehensive questionnaires and history forms will be completed and review with Dr. Nieves. Following the history portion of the evaluation Dr. Nieves will conduct a Physical Examination and review all records provided for which it is imperative to have all records and films brought to Dr. Nieves prior to the evaluation. If the individual was referred for diagnostic and treatment and a doctor / patient relationship is established Dr. Nieves will proceed to review diagnostic study findings and treatment plan with the patient..
Who can refer IME’s, Impairment Rating Evaluations and other Medico-legal work to Dr. Nieves?
Insurance companies, Attorneys, and other providers that treated the examinee and require an Impairment Rating (might need approval from Insurance Company). Dr. Nieves does work for both plaintiff and defense with honest, accurate and thorough medico-legal documentation.
What to expect during the Electrodiagnostic Medicine study (Emg/ Ncs)?
There are no restrictions in activity before or after the evaluation. The examination takes 20 to 60 minutes. On the nerve conduction study portion of the evaluation electrodes are placed on your skin and several brief electrical impulses of varying strength are applied to the nerves. Measurements are then taken to established how well the nerves send messages and how fast or slow the nerves are conducting. Specific nerves in the arms or legs are evaluated in this way. On the needle Emg portion of the test a thin sterile needle electrode is inserted into specific muscles to detect electrical activity. The nature of your disorder will determine the number and location of the muscles to be examined. During the examination electrical activity from the muscles is displayed on the screen and broadcast over a speaker and interpreted by Dr. Nieves. The needle is discarded after the examination is completed. The test results are then interpreted by Dr. Nieves and a report is generated.
Preparation for the Emg/ Ncs?
Bring comfortable clothes and clean hands and feet. Do not apply moisturizing cream to your hands and feet on the day of the study. If you have a cardiac pacemaker or take Coumadin (Warfarin) or any other blood thinning medication notify Dr. Nieves before the examination since the examination may need to be modified for you.
Who performs the Electrodiagnostic Study (Emg/ Ncs)?
Dr. Nieves performs the whole study (Nerve Conduction study and Electromyography) himself. Dr. Nieves is Board Certified in Electrodiagnostic Medicine and Fellow of the American Association of Neuromuscular and Electrodiagnostic Medicine. Dr. Nieves has over 20 years of experience performing and interpreting Electrodiagnostic Studies (Emg/ Ncs).
Who can refer an individual for Electrodiagnostic Study with Dr. Nieves?
